    TB questionnaire

    All new incoming students (freshmen, international, and transfer students)are required to complete the online  and have a TB skin test if indicated by the questionnaire. The test must have been performed in the United States or Canada within the last 6 months. Failure to complete the Questionnaire will result in a hold being placed on your account preventing you from registering for the following term.

    The dates listed below are deadline dates for TB Questionnaire toP be completed, otherwise a Wellness Center Hold will be placed placed on your student account.

    Fall Term: October 3 

    Winter Term: February 13 

    Summer Term: June 13  

     

    What are the 2 types of ways to get a TB Test?

    1. The first is a skin test.  If you schedule on a Monday, Tuesday, or Wednesday then 48 afterwards  which could be Wednesday, Thursday, or Friday.  You can have it read by one of our Nurse Practitioners during the Wellness Center normal business hours.
    2. The second is a Blood draw which only requires one appointment.  This option is more costly but more accurate.

    Bacille Calmette-Guerin (BCG)

    If you are a student who has received the BCG which is a vaccine for (TB) tuberculosis disease.  The vaccine can cause a false-positive TB skin test reaction.  The Wellness Center recommends getting a TB blood draw.  This will ensure a more accurate result.  If it does come back abnormal, further testing will be required before the Wellness Center can process the TB Questionnaire.

    Parents/Legal Guardian

    If your student is a minor at the time time the TB Questionnaire is due, please make sure you sign the document.  You will receive an email from "WellnessCenter_DF@udmercy.edu".  Please check your spam folder to make sure you received the message.  The TB Questionnaire cannot be processed without a parent or guardian signature.

     

     

     

     

  • Student Health Insurance

    All students who will be living in the Residence Halls must have documentation of health insurance.

    Find more information about student insurance, or to waive the plan offered by the University.

    The Wellness Center accepts the following insurances: Aetna, BCBS/BCN, Cigna, Humana, Priority, and UHC.

    If you are out-of- state or a student that does not have the following insurance plans (Aetna, BCBSMI, BCN, Cigna, Humana, Priority, and United Healthcare) please contact your insurance company directly to inquire about what you as parent/guardian or your student needs to do by using an insurance that is not listed especially if the student is planning to live in the residence halls.

  • Health Programming Request

    This form is used by students and employees at the 黑料社区app to request an educational health program from the Wellness Center.  We are best able to make your programming request if it is submitted as far in advance as possible.  We request at least a 30 days' notice from you.  Our normal business hours are between 8:30 am and 5:00 pm Monday through Friday.  Please give the Wellness Center 2 business days to get back to you with our answer on whether or not we can participate.
